Review, sign, and send us the consent form that applies to your situation. Consents can be sent via email or post.

Consent form for a family member who is interested in blood donation but unable to legally give consent.

Consent form for anyone at any time to make their future brain donation wishes known; and for family members of anyone nearing the end of life.

Once consent is received, we will ask you to provide demographic information, family history, and relevant medical records via a questionnaire and/or by signing a records release form.

Our team will pre-arrange everything for sample donation and shipping. We have financial support to cover all costs of the donation procedure including collection, shipping, and storage.

Regarding brain donations, please discuss your biobanking wishes with your next-of-kin and healthcare providers. To be useful for research, the donation needs to occur as soon as possible after death (ideally within 12 hours). Arrangements will be made in the days/weeks leading up to death with additional consent forms signed by your next-of-kin. We will prepare everything in advance to ensure the process goes smoothly without delay to funeral plans and with little burden on your loved ones.
